Abstract: Employee #1 was operating one of two track-hoes on a loading dock excavation/demolition project. He was primarily excavating the dock, while the other operator broke up and loaded the concrete. Employee #1 was starting to excavate an old 14 ft long by 4 ft wide by 2 ft thick concrete footing that had been previously broken up with a drop ball. He grabbed the footing near the center with the bucket and proceeded to lift it to the ground level of the track-hoe when the footing shifted unexpectedly and fell, striking Employee #1 in his cab. He sustained a compound fracture of his left leg, torn knee ligaments, a crushed heel, and lacerations of both his legs and feet.
