Abstract: Employee #1, age 19, was unloading room air conditioning units through a window into a hotel under construction. He was standing on a three-sided box designed to raise and lower scrap material that was resting on the forks of a forklift. Employee #1 was taking off the A/C units without regard to maintaining an evenly balanced load in the cage. The load became unbalanced and the box, along with Employee #1, fell 22 ft 3 in. to the ground. The cage was not secured to the forklift. The distance to the floor from the sill was 2 ft 10 in, and the sill was nearly 1 ft across.
