Abstract: Employee #1, a maintenance worker, was repositioning a drive belt on its pulleys. The motor and pulley were on a 30 ft high walkway on the outside of a fly ash silo at the mixing plant, where trucks were loaded with the components of cement. The motor was controlled from elsewhere in the plant in response to the amounts of the various constituents being loaded into each truck. As Employee #1 moved the drive belt, the motor started in response to the activation of a distant sensor. His thumb became caught between the belt and the pulley, amputating the tip from the distal interphalangeal joint. Employee #1 had not followed the company's lockout/tagout program, which was to interrupt power at the breaker and tag the switch, or to shut the plant down entirely.
