Abstract: On May 31, 1995, a 2-liter plastic tea bottle was injected with liquid nitrogen and exploded. Pieces of the bottle struck Employee #1, who sustained facial fractures that subsequently required surgery. During the investigation the plant went on strike and the machine operator was fired. The plant bottles carbonated and non-carbonated drinks on a continuous line in bottling room 2. The containers are plastic. Malfunctions in the metering device had occurred prior to Employee #1's injury and a report was later issued documenting management involvement. The line was not shielded against container rupture, and Employee #1 did not have facial protection. Both items have been addressed. Citations were issued as follows: energized electrical panel open, serious citation; failure to shield bottling line, accident-related. T8CCR 2340.1, 3273(h).
