Abstract: At approximately 8:00 p.m. on June 13, 1997, Employee #1, of Midwest Fabricating Company, was using a Walsh Press and Die Company punch press machine #20 to make J-hooks. His right index finger became caught in the point of operation and was partially amputated. He was transported to the hospital, where his finger was surgically reattached. Employee #1 had been wearing a restrainer, or pull-out device, on his hands, but had not made any adjustments to safeguard his hands. The employer was cited for a serious violation of T8CCR 4208(d)(4).
