Abstract: At approximately 4:30 p.m. on March 7, 1997, Employee #1, of the Jaeger Broach Company, was using a National American vertical broach machine #7 to make small parts. A coworker and her friend were playing darts nearby, which diverted Employee #1's attention and caused her left little finger to become caught in the machine and be amputated. After an investigation, the employer was cited for a serious violation of T8CCR 4184(b). In a separate incident, at approximately 10:00 a.m. on April 21,1997, Employee #2, also of the Jaeger Broach Company, was using a Pioneer horizontal broach machine, model #H-320SPFL, serial #C-5043, to make small parts, such as control knobs. She loaded a part with her right hand and pushed the START button with her left hand. Before she could pull her hand out, the broach pushed the part into the spline mechanism, amputating her right middle finger. After an investigation, the employer was cited for a serious violation of T8CCR 4184(b).
