Abstract: Employee #1 was engaged in a maintenance operation at a cultured stone manufacturing plant that involved a double "V" vibrator rocker auger, double "V" line #1-2, and a V-mold constructed in-house. He was standing in front of the machine attempting to tap a protruding key back into the keyway of the sprocket when a coworker accidentally turned on the line, crushing the fingers on Employee #1's right hand between the chain and sprocket. The causal factors contributing to this accident were the guard on the chain drive had been removed; lockout procedures were not followed; and the equipment stopped only with push-pull line-stop switches.
