Abstract: Employee #1, an electrical apprentice, was installing a smoke detector on the ceiling of a garage. He was standing on a borrowed 10 to 12 ft portable metal extension ladder, which he had propped up against the hinges of the automatic garage door box. He finished installing the detector and was descending when the ladder slipped out from under him. Employee #1's left hand became caught in the V-shaped door opener support mechanism, resulting in severe lacerations to his little finger, ring finger, and thumb. The ladder had not been tied off, blocked, or otherwise held in place.
