Abstract: On April 22, 1995, Employee #1, who works for a company that manufactures metal components for the air bags in Mazda vehicles, was unjamming a Mazda Press A punch press machine. The machine's magazine tray had jammed during production. Employee #1 removed the electrical source of energy to the machine but failed to disengage the pneumatic energy supply before attempting to unjam the machine. As a result, his little fingertip was amputated. There were no witnesses to the accident. Employee #1 was not trained on lockout/tagout or energy control procedures until after the accident.
