Abstract: At approximately 11:15 a.m. on October 31, 2001, Employee #1, a cabinet maker/bench carpenter, was operating a pinch roller machine to make a cabinet. He amputated two fingers and lacerated and crushed two finger tips, while feeding two short pieces of lumber through the pinch roller machine. The fingers became lodged between the underside of the board and the lower pinch roller. The pinch roller was properly guarded and equipped with an emergency stop button. Employee #1 was taken to the emergency room at Kaiser Hospital and released that day.
