Abstract: Employee #1 was lubricating and adjusting three conveyor belts at a sand and gravel operation. The foreman was instructing Employee #1 through hand signals to retract a belt below the rock crusher while it was running. As he was accessing the tail pulley, Employee #1 was distracted by a load of rock being dumped onto the grizzly directly above the tail pulley on which he was working. As he looked away, Employee #1 dropped his right arm into the take-up pulley. He became caught up to his shoulder and his leg became wedged between the conveyor system and the grizzly backing plate. Employee #1 suffered rubber friction burns to his right arm and multiple fractures to his right leg. Sometime earlier, someone had removed the cover guard to the tail pulley.
