Abstract: On September 25, 2002, Employee #1, a journeyman carpenter with over 12 years of experience, was working at a remote single-family residential construction site. He was using a Dewalt 12-inch chop saw, Model Number DW708, Serial Number 24712, to cut 2-inch by 6-inch lumber when the tip of his left index finger contacted the blade and was severely lacerated. He was hospitalized and the fingertip later had to be surgically amputated.
