Abstract: Employee #1, a supervisor, was in the warehouse talking to the manager. They were behind the pedestrian walkway at the intersection at column #19. The supervisor and manager regularly walked through the warehouse. A coworker driving a Raymond forklift carrying two pallets of Dewalt Drill Kits (one on top of the other) was approaching the intersection where Employee #1 was standing. When he reached the intersection, he stopped. Although the two pallets, which weighed almost 3,000 lb., were wrapped in tight plastic, the boxes inside could slide around a little and the forklift driver was constantly checking the load behind him. When he started to make a left turn into another aisle, the top pallet started to fall and eventually both pallets fell to the floor. The falling boxes flew into the air and some struck Employee #1, cutting and bruising her back, legs, ankles, and feet. She also sustained ligament damage and two broken bones in her right foot.
