Employee #1 was walking backward as he applied hot tar with a mini-mopper to a flat roof 13 ft 6 in. above the ground. He misjudged his location on the roof and fell backwards onto concrete pavement. He carried the mini-mopper over with him and it landed on his face. Employee #1 sustained facial fractures and 2nd- and 3rd-degree burns to his face. He also sustained damage to his left eye and fractured two vertebrae. Employee #1, the owner of the company, has more than 30 years of roofing experience. No fall protection equipment was in use at the time of the accident.
