Abstract: At approximately 5:00 p.m. on July 21, 1998, Employee #1, age 19, of Tom Day Tree Service, was loading a tractor onto the bed of a trailer. He was chaining it down when the chain binding lever slipped and struck him on the forehead. Employee #1 suffered a fractured skull and was hospitalized for six days; he has since recovered and returned to work at the same job. The chain binding hand lever was inspected, but no defects were found.
