Abstract: At approximately 12:30 p.m. on March 19, 1997, Employee #1 was using a Tommy Gun plaster pumping machine, A3 Series, to plaster an apartment building. A coupling broke loose, allowing the hose to whip around and strike Employee #1 in the mouth. He was knocked unconscious and suffered a broken nose, lacerations on his upper and lower lips, and the loss of several teeth. He was transported by paramedics to the hospital.
