Abstract: At approximately 2:30 p.m. on May 19, 1997, Employee #1, a carpenter, was working at the tee line canopy construction site at the Chuck Corrica Golf Complex in Alameda, CA. He was stepping off a forklift boom over the canopy shroud and onto a pallet of material when a portion of the canopy collapsed. Employee #1 lost his balance and fell forward onto a completed section of decking, which partially broke his fall. He continued to fall 12 ft, striking his head on the concrete surface. Employee #1 died as a result of his injuries.
