Abstract: At 3:00 p.m. on July 14, 1993, Employee #1 started work at the RMC Lonestar's Bonny Doon limestone quarry. Water with additives is continuously applied in order to maintain and control dusty conditions on the main haul road. Employee #1 came off branch #3 onto the main haul road and moved into the center. The 15 to 20 foot wide center portion of the haul road is normally undisturbed; therefore, there is a tendency for the material being applied to control dust to concentrate and build up, creating a very slippery road and surface condition as more water is applied. As Employee #1 was descending down that portion of the haul road his right rear set of dual truck tires started sliding. As he started to make the required left turn near the lower end of the haul road, the rear end of his truck started rotating counterclockwise and continued sliding across the haul road into the opposite lane of traffic. The right rear corner of the haul truck struck the outside safety berm, causing the truck to roll over onto its right side. Employee #1 regained consciousness as his supervisor climbed onto the truck to remove him. Employee #1 was calmed and flown to Stanford Medical Center for stitches for his head laceration and overnight observation. Employee #1 had received 2 days of training under favorable conditions for operation of the haul truck. He stated that the haul road was very slippery earlier in the day.
