Abstract: At approximately 11:30 a.m. on April 12, 1996, Employee #1 was walking from the northeast corner of a roof to the southwest corner. At the same time, a coworker was using a skill saw to cut a 6 ft by 4 ft air conditioning hole. The coworker's saw became unplugged with several more inches to go before the hole was completely cut through. Employee #1 spied the extension cord and was trying to plug it back into the circular saw when he stepped onto the nearly open air conditioning hole. He fell approximately 18 ft to the ground. Employee #1 sustained massive head injuries and died four days later.
