Abstract: At about 12:15 p.m. on October 12, 1994, Employee #1 was checking a mechanical platform on the roof of a building under construction. He climbed a stairway to the mezzanine floor, which was about 14 ft high, and then used an extension ladder to climb approximately to the roof's doorway, about 4 ft above the mezzanine floor. As Employee #1 started to descend the ladder, it slipped and he fell. The ladder had been set up about 2 to 3 ft away from the hole in the mezzanine floor for another stairway that had not been built. Only one side of the hole had a cover or guardrail. After hitting the mezzanine floor, Employee #1 rolled into the hole and fell another 14 feet. His injuries were minor and he was not hospitalized.
