Abstract: Employee #1 was approximately 12 ft above the ground, hanging Visqueen tape, when the extension ladder on which he was standing slipped out from under him. He fell, landing on his hands and knees, and striking his forehead on the ladder. Employee #1 suffered a head laceration that required 12 stitches to close.
