Abstract: Employee #1 and coworkers had started a fracturing job on an oil well. Employee #1 was operating a nitrogen pumping unit while standing next to another pumping unit. When the pressure of the adjoining pump reached approximately 3,200 psi, a union in a high pressure line failed. The escaping gas from this line, which had been tested to 8,000 psi, threw Employee #1 into the side of a flatbed trailer, causing severe chest trauma from which he died.
