Abstract: Employee #1 was climbing a 24 ft metal extension ladder to access the top of a battery room to install HVAC angle irons. He was on the top 12 ft section of the ladder when it overturned and he fell to the floor, sustaining multiple fractures to his arm. The ladder did not have the required safety feet, the upper section of the ladder was not secured, and the ladder was too short for the required pitch and extension above the landing.
