Abstract: Employee #1 was cleaning dirt from around an 8 in. blind flange that was under pressure. He had been instructed by a journeyman pipe fitter that after cleaning the fitting he should wait for the pipe fitter to return with the fittings. Instead of waiting, Employee #1 started removing the bolts on the blind flange without draining the line pressure through the existing drain plugs. The flange flew off and struck him below the knee, breaking his leg below the knee. Employee #1 has completed 2 years of a 5 year apprentice program.
