Abstract: At 11:30 a.m. on May 5, 1993, Employee #1, of Sentinel Sports Products, was operating a shaping machine used to cut foam for bellyboards. The board being cut was approximately 18 in. by 24 in. by 3 in. high. The foam was placed on a table and held firmly, then guided through a vertical saw blade to cut the foam to the shape of a bellyboard. The saw blade was adjusted to the width of the foam. Employee #1 stated that his hand slipped while holding the foam and his hand went into the blade, amputating his right ring and middle fingers below the nail area. Employee #1 stated that he was trained, but was not performing his normal job task. No violation was established for the accident; however, a citation was issued for not reporting the serious injury within 24 hours.
