Abstract: At 11:30 a.m. on August 23, 1995, Employee #1 was using an automated Bliss C-75 punch press to punch out parts on a 1 1/2 in. by 1/8 in. steel band material. The band material jumped out of the track, causing the press to jam and stop. When Employee #1 reached in to adjust the material that was out of the track, this pushed the end of the material into the sensor at the end of the line, activating the press. The punch came down on his right hand, crushing and amputating four of his fingers just below the knuckles. Employee #1 was taken to Bear River Hospital for emergency care and then transferred to Mckay Dee Hospital in Ogden. Employee #1, who had 10 years of experience with this machine, failed to lock out or shut down before trying to fix the jam.
