Abstract: Employee #1, an iron worker for Midwest Steel Erection, Inc., Oakland, CA, was changing out steel bracing atop the cap of a bent underneath a freeway connector as part of a seismic retrofit of the freeways. The cap of the bent was between 55 and 60 feet above the ground. Employee #1 and his partner were moving in and out of the basket of a Grove AMZ66XT aerial device. They both wore safety belts and lanyards. When they were working in one spot, they tied off; when they were moving around atop the cap or in and out of the basket, they unhooked and walked around without fall protection. Apparently, Employee #1 was either moving across the cap with a bucket of bolts, or trying to get back into the basket with the bucket of bolts, and lost his footing and fell to the ground either off the bent cap or over the back of the basket. He later died of massive head and chest injuries.
