Abstract: Employee #1, an automatic fire sprinkler fitter for a company that installs fire protection systems, was working on the roof of a two-story building. He was preparing to go down a metal ladder through a roof opening and had his left hand on the ladder. When he grabbed a support for the guardrail system around the roof opening, the support came loose, causing Employee #1 to lose his balance. As he pivoted around the ladder, he hit his left leg against the roof opening and fractured his femur. He was hospitalized. The guardrail system consisted of 1 1/2 ft box metal for uprights, with 5/8 in. cable for the rails. The uprights were clamped onto the top and underside of the concrete roof opening.
