Abstract: Employee #1 was assigned to turn off a portable 20 hp water pump used to wash out juice tanks. The pump had been operating with limited water flow, and the trapped water in the pump had become heated. Three weeks before the accident, Employee #1 had been trained to open two pressure relief hoses before opening a coupling or threaded fitting. Instead, he opened only one relief valve and then the coupling. Pressurized hot water sprayed onto his midsection and lower body. Employee #1 was admitted to the hospital, where his burns became infected. He died at the hospital on November 5, 1999.
