Abstract: At 11:30 a.m. on December 12, 1995, Employee #1, a benchman for Imperial Radiator Shops, Torrance, CA, was removing a sensor patch from a boat gasoline tank when it exploded. Employee #1 was struck by the tank and hospitalized with a fractured leg. He stated that the tank had been soaped and was thought to be all right. Employee #1 was not trained about the necessity for sparkproof removal of gassy tanks and the complete removal of fumes from fuel tanks before hot work begins. Lack of documentation concerning these issues resulted in citations for T8CCR 3203 (a)(3) (failure to establish and maintain a fire prevention procedure) and T8CCR 4848 (a)(4) (failure to adequately train employees in the use of combustible gas meters and cutting torches and the related hazards of working with these tools). Employee #1 appeared not to understand and connect the principles involved that resulted in this accident. Photos of his injured leg, the shop, and the tank were taken. A coworker in close proximity to the accident was unable to fully identify the events preceding the explosion.
