Abstract: Employee #1 was patting a running warper beam to check for high spots when his hand was caught in the running nip point of a Jack Spool warper. His body, from head to hips, was pulled into the machine between the warper beam and the warper beam pressure roller. The warper beam pressure roller created 580 psi against the warper beam. The movement of the rotating warper beam fractured both femurs when they struck the warper beam support cradle. Because of the pressure of the press roller, Employee #1 suffered a fractured jaw and contusions and abrasions to the upper body. The practice of patting a running warper beam violated known, written, and enforced company safety rules. Actions taken on previous company safety violations were well documented. This accident was the result of employee misconduct.
