Abstract: At approximately 1:00 p.m. on Monday, June 13, 1994, Employee #1, a press operator, was operating a Rockford 100 ton full revolution mechanical power press, #8-S. He reached into the point of operation with both hands to retrieve a stamped ladder rung part from the lower die. Simultaneously, the mechanism brake pad sheared off its mounting rivets, and, as the ram cycle completed at the top, the entire ram rolled over and began a gravity fall. The ram caught Employee #1's left and right thumbs between the upper die and two spring-loaded fixtures in the lower die. His thumbs were saved from amputation because of the give within each fixture but he required hospitalization. The press had been purchased on sale five months before the accident. No initial maintenance inspection of all parts and equipment had been performed. Employee #1 had been trained as a pressman and had been producing for five weeks prior to the accident. There was a significant lack of professional competence on the part of management and maintenance/production leadership.
