Abstract: At approximately 11:30 a.m. on July 22, 1993, Employee #1, a fabricator mechanic who repaired semi trailers for Apex Trailer Services, Inc., Jeffersonville, IN, was in bay #27 preparing to repair a large Peterbilt over-the-road tractor. He had just participated in a brief strategic meeting with the shop foreman and the driver of the tractor trailer, who was highly experienced as a truck driver and also worked with Employee #1 as a fabricator/mechanic. Apparently some miscommunication occurred, because as the driver backed into the bay, the over-the-road tractor struck Employee #1 and crushed him against a trailer on which he was installing wiring for lift gates. Employee #1 suffered three fractured ribs and a fractured clavicle, all on his left side. He was hospitalized overnight and released. Evidently, Employee #1 assumed that the truck would not need to back up so far into the bay, and the truck driver assumed that Employee #1 would not return to his work station in front of the trailer as quickly as he did.
