Abstract: At approximately 11:20 a.m. on September 13, 1995, Employees #1, #2, and #3, who worked for an excavation and construction company, were in an unshored portion of a trench, standing about six feet west of a trench shield. The trench was approximately fifteen feet deep by four feet wide and eighty feet long. The excavation cave-in completely covered Employees #1 and #2; Employee #3 was buried up to his waist by debris. Employee #1 and #2 were completely covered to a depth of at least two feet above their heads and were asphyxiated. They were killed. Employee #3 was buried up to his waist and sustained multiple body strains and contusions, but was not hospitalized. The Employees were not being closely supervised at the time of the cave-in. Employees had not been thoroughly trained on the safety procedures when working with a trench shield. The excavation also contained water at the time of the cave-in, which contributed to the cave-in condition.
