Abstract: At 3:40 p.m. on February 26, 1997, Employee #1 was on a crew that was removing outdoor electric store signs. They were using a truck-mounted, winch-powered extension ladder, with a personnel platform at the top to reach the signs. Employee #1 was on the approximately 40 ft ladder when he unlatched the safety catch. All three sections of the ladder collapsed together, trapping Employee #1's fingers between the platform and the ladder when the sections struck the base support. Two of his fingers were amputated, and he was hospitalized for surgery and treatment.
