Abstract: Employee #1 was hand-feeding sheets of 26 gauge galvanized steel, measuring 38 inches long by 18 inches wide, into a continuously running roll press to form ducting. The barrier guard had been removed several months prior to the accident. Employee #1's gloved hand was pulled between the rolls as he was feeding a sheet. He sustained a crushing injury to his left hand. He was transported to the San Ramon Medical Center, where his ring finger and the tips of two other fingers were surgically amputated.
