Abstract: At approximately 10:00 a.m. on August 19, 1993, Employee #1, a maintenance lead with over 5 years of experience at his present job, suffered a serious injury while servicing an Acme electric spot welder. He was attempting to realign the arm of the micro switch on a energized welder. Employee #1 placed his finger under the switch and activated the foot pedal to elevate the "dog ear" located at the collar of a shaft attached to the foot pedal. The dog ear acted as a knife and severed his finger. Employee #1 could have used a screw driver or another extension tool below the dog ear or loosened the micro switch from the shaft instead of placing himself in danger. A violation of 3314(a) was found.
