Abstract: Employee #1, an apprentice mechanic at an auto repair facility, was told to change the fluorescent lights in the office. He used a 14 ft plastic extension ladder with metal rungs to reach the office ceiling. The unextended ladder, which had been stored outside in the rain, was placed at an approximately 45 degree angle with the floor. Employee #1 changed the lights and handed the old bulbs down to a coworker. He was descending when the ladder's feet slid out; the top of the ladder came off the office wall and dropped to the floor. Employee #1 rode the ladder down, suffering fractures to his knee, elbow, and wrist. The cause of the accident was placing the ladder, with wet feet, at such a shallow angle. This did not provide secure footing and it allowed the ladder to slide.
