Abstract: Employee #1 was helping to move steel by working as the ground hook-up person, (the person who attaches the lift strap to the lift truck). He was walking in front of a moving, empty forklift truck, supplied by a local manufacturer, that had its forks elevated approximately 10 ft. The right front wheel of the forklift was driven through a pothole, and the left fork, which was unsecured, detached and fell. The fork struck the ground, flipped, and struck Employee #1 in the back. He was taken to the hospital by ambulance for a checkup. He was released the same day.
