Abstract: At approximately 9:30 a.m. on May 21, 1999, Employee #1 was working as a helper for Lowpensky Molding in San Francisco. As he was feeding lumber into a Weinig molding machine, he was told several times to pay attention to the curve of the lumber. The guards on the molder were missing, which made it easier to see whether the lumber was up against the fence or not as it traveled. Employee #1 accidentally placed his left hand into the rotating head, and his left index and middle fingers were amputated. Emergency services was called, and he was transported to San Francisco General Hospital.
