Abstract: Employee #1 was a laborer who had worked at Kodiak Construction for two days. At approximately 1:00 p.m. on June 16, 1999, he was demolishing scaffold staging when he fell 6 ft 3 in. from a wall or platform. Employee #1 sustained a small cut on the side of his head and an injured wrist. He went to Kaiser Permanente in San Francisco, where he was examined and discharged. On July 2, 1999, after discovering that he had actually fractured his wrist, he returned to the hospital for surgery. Pursuant to the accident investigation, it was recommended that (1) the employer have a method for ensuring employees' compliance with safe work practices as part of its Injury and Illness Prevention Program )IIPP); (2) the employer conduct an initial job site survey to identify all hazards associated with each specific job site and develop specific methods for controlling the identified hazards; and (3) the employer have training and retraining ,as well as instruction, for its employees. No accident-related violations were found in connection with this incident.
