Abstract: At 7:36 a.m. on July 17, 1997, Employee #1 was performing his regular job of repairing the two main units of the generator at PG&E at Hunters Point Power Plant in San Francisco. The casing of the generator he was working on had been pressurized the day before to detect the plug leaks in the coolant tubes. Employee #1 attempted to remove the access cover by removing 23 of the 24 bolts where it was under pressure. The pressure from inside caused by the casing stripped the remaining threads off the bolt (the last bolt was still partially threaded). The cover blew off and struck Employee #1 in the head. The force was so great that it damaged the guard railing on the turbine deck.
