Abstract: Employee #1, an experienced employee of C.J. Hammer Machinery Co., was using a forklift to reposition two sections of 20 ft by 8 in. ID down-well pipe so that their coupling ends would be to the rear of a service truck. Employee #1 removed the pipes from the truck and set them down so that he could move the forklift to the opposite side and position himself to reload the pipe with the correct orientation. He began backing up the forklift to the right as he looked back over his left shoulder. The right rear wheel (steering) went into a depression, said to be 3 to 8 in. deep and 18 to 20 in. in diameter, causing the forklift to tip over on its right side and pin the employee under it. Employee #1 suffered skull and facial fractures and was taken to Kaweah Delta District Hospital, where he was admitted and treated.
