Abstract: At approximately 1:40 p.m. on June 10, 1999, Employee #1, a supervisor at the Holiday Rock Company, Inc., with 29 years of aggregate mining experience , was directing the removal and transport of a pipe. The pipe was chained to the bucket of a front-end loader. When the loader operator lifted the bucket, the pipe swung around and fell on Employee #1's left foot. He sustained crushing injuries and was hospitalized for 23 days. The exact cause of this accident in not known; it was probably the result of operator error and of not securing the load properly. Employee #1's failure to stand clear of the pipe as it was lifted was a contributing cause.
