Abstract: At approximately 6:15 p.m. on August 31, 1990, Employee #1, who is normally assigned as a truck driver, was loading newspaper color inserts. These banded inserts are sent to the loading dock by the newspaper conveyor system in the hall. The employee attempted to remove a broken bundle of inserts but his right hand was suddenly pulled downward into the 1 in. gap between the two 24 in. wide by 2.5 in. deep belt-covered rollers. He said that the forward belt went into reverse for unknown reasons. He suffered lacerated wounds to his right hand with arterial and nerve damage for which he was hospitalized. A general citation was issued for the violation of T8CCR 4070(a).
