Abstract: At approximately 4:21 p.m. on January 17, 1990, one of the large, 16 ft long metal roll-up doors at Redwood Construction Trucking in Ukiah, CA, was damaged by a vehicle entering the shop. Employees #1 and #2 were going to remove the door and repair it so that the shop could be secured for the night. For a work platform, they placed a 3 1/2 ft by 4 ft by 3 ft shop table on the forks of a Clark forklift and elevated it approximately 9 ft above the concrete floor. Employee #1 had removed the belts from the bracket on the south end of the door and was loosening the belts on the north end when the door started to unroll. Both employees were knocked off the elevated table. They were transported to Ukiah Medical Center; where Employee #1 was hospitalized with two broken heels and Employee #2 was treated and released. The work platform (table) was not equipped with a guardrail, nor was it secured to the forks.
