Abstract: At approximately 12:50 p.m. on May 19, 1994, Employee #1 was "cratexing" parts on the number 8 polishing machine in the polishing department. Employee #1 noticed the wheel was getting very small, so he asked his supervisor for a new wheel. While waiting for the new wheel, Employee #1 turned off the polisher. Employee #1 was still holding a part in his hand with the part against the wheel while the wheel was rotating to a stop. Employee #1 was wearing cloth mesh gloves. The ravelings from the glove caught on the still rotating threaded arbor end, pulling the employee's right hand onto the arbor. Employee #1's right ring finger was amputated below the second joint. Employee #1 was treated at Valley Hospital Medical Center and then transported to surgery. Employee #1 was released the same day.
