Abstract: At about 7:00 a.m. on May 16, 1994, Employee #1 was instructed to go up on the already installed section of an aluminum metal roof on an elementary school under construction. He took up the required tools to finish seaming the roof. The pitch of the roof was approximately 2.6 to 12 or less. Employee #1 was approximately 20 feet from the edge of the roof when he realized how slippery the roof was from the rain the night before. He was about to come down from the roof when he started to slip and slid backward down the metal roof to the edge. Employee #1 fell 10 feet to a dirt section in the courtyard, lost his balance, and fell backward again, putting out his right arm to brace/catch himself. His right hand landed on the concrete. The force of falling and bracing himself on the concrete broke his right arm between the wrist and the elbow. Coworkers came to his assistance. 911 was called and he was taken to Hartford Memorial Hospital where he stayed for 3 1/2 days. Employee #1 was not wearing fall protection.
