Abstract: Employee #1 was being trained to operate an edger chipper at a saw mill. He had been training on the machine for 1 week under close supervision. His supervisor was called away from the operation for a short period of time. Employee #1 (for no explainable reason) walked approximately 5 ft from his normal work station to the machine's unguarded feed rollers. Employee #1 came in contact with the rollers and his upper body was pulled into the rollers. Employee #1 suffered severe contusions and lacerations to the upper body as well as a right hand amputation; he died.
