Abstract: At approximately 12:30 a.m. on May 20, 1991, Employee #1, a machine operator at Milan, Inc., was loading plastic tubing into a Farris universal take-off machine, serial #0650V7303. He inadvertently reached into the in-running rollers and suffered compound fractures of his right ring and middle fingers. He was treated at the Intercommunity Hospital in Covina, CA, and released the same day.
